What are the required documents for applying for UIF maternity benefits?

When applying for maternity benefits under the Unemployment Insurance Fund (UIF), you will typically need to provide certain documents to support your application. Completed UI-2.8 Form: This is a form that needs to be taken to your bank and completed this authorized UIF to pay our UIF Benefits into your bank account

 

Identity Document: A copy of your valid South African identity document or passport is usually required as proof of your identity. - Your ID must be updated as per home affairs records

 

Birth Certificate: You will need to submit the birth certificates or a letter from a medical practitioner confirming your pregnancy and expected due date. The medical certificates should include the date of commencement of maternity leave and the expected date of delivery.

 

UI2.3.  Application for Maternity Benefits. Part of the UI.2.3 must be completed by your Medical Practitioner confirming your pregnancy and expected due date. The medical certificates should include the date of commencement of maternity leave and the expected date of delivery.

 

UI.19 : You may be asked to provide proof of your contributions to the UIF, such as payslips or a UI19 form issued by your employer.

 

Salary Schedule Form: This form is needed to provide DOL with your yearly salary adjustments and contribution amounts.
